Al Gauvin

Al Gauvin founded Blossom: Modern Home Care Solutions of Western New York (then CCOR) in 1997 as a solution for families looking for quality home care. He applied his passion and knowledge of home care to developing Blossom into an agency that makes a difference in our clients’ lives every day. His dedication to helping individuals maintain their independence at home has led Blossom to expand to four offices and 18 counties throughout Western New York to make services more accessible to those in need. Even though Al retired in 2018, he continues to be involved in the growth and progression of the company, bringing his dedication to everything that they do.

Emily Briggs
CDPAP Manager

Emily graduated from Bowling Green State University in 2016 with a bachelor’s degree in Health Administration. She started with Blossom in 2017 as the CDPAP Intake Coordinator and quickly rose to the role of CDPAP Manager in the fall of 2018.  Emily oversees the day-to-day operations of the Consumer Direct Personal Assistance Program (CDPAP).

Santali Vazquez

Clinical Director

Santali Vazquez graduated from Nazareth College with her Bachelor’s degree in Nursing in 2016. She joined the Blossom team in the summer of 2017 as an HCSS Supervisor. In her four years working with the company, she has continued to develop in her different roles as Patient Services Manager of ENY and Assistant Clinical Director. Santali has a passion for providing quality care and enjoys working with the clinical team. In her role as Clinical Director, Santali oversees and supports the LCHSA nurses. She is also a member of the Community Health Care Services Foundation.

Molly Ford
Director of Finance

Molly Ford started with the company in September of 2011. She has had a few roles during her tenure at Blossom, first as the Accountant for the agency covering all aspects of finance.  She also held the position of Chief Compliance Officer for approximately a year and a half which added to her skills and greater knowledge of the home health services. In 2017, she returned to the finance department as the Director.  She has over 20 years of experience in the field of accounting

Colleen Boice

Director of Health Homes

Colleen Boice has been with Blossom since 2014, working in a variety of roles including service coordination, intake, and business development. She has a Bachelor’s degree in social work from Nazareth College and a Master’s degree in special education from Roberts Wesleyan College. Currently, she utilizes both her expertise and her compassion as the Supervisor the Health Homes Department, which services adults and children with complex medical and/or behavioral issues. Additionally, her various roles at Blossom give her a thorough understanding of the company’s programs and help her to better serve Health Homes clients and their families.

Brooke Findlay

Marketing Manager

Brooke Findlay graduated from SUNY Brockport in 2015 with a bachelor’s degree in Organizational Communication. Brooke began her work at Blossom in the fall of 2019 as the Marketing Support Specialist and steadily grew into her role of Marketing Manager, which she stepped into in the summer of 2021. Brooke uses her knowledge of effective communication and eye for design in the day-to-day operations of overseeing the Marketing department. Outside of Blossom, Brooke is an active member of the Greater Rochester Area Partnership for the Elderly, sitting on several committees, as well as co-chairing one.

Liza Torres

Liza Torres brings over 20 years of home care experience to Blossom. Her lengthy experience covers multiple levels of service in the health care industry. On the administrative side, Liza has overseen staff for both a Certified Home Health Agency (CHHA) as well as a Licensed Home Care Service Agency (LHCSA). She has knowledge and experience from a hands-on level of home care all the way through operations and management, making her an important part of the Blossom family since 2014.

Chris Gauvin
President & CEO

Chris Gauvin graduated from SUNY Buffalo in 1999 and has worked with Blossom: Modern Home Care Solutions of Western New York since its beginning in 1997.  Chris has helped the company grow from a small business operating in a one-room office to the corporation it is today – an employer of almost 700 individuals in Western New York.  As a vital member of the Blossom family, Chris has worked in various roles over the last 20 years, ranging from QA and IT up to running day-to-day operations.  Chris assumed the role of CEO in July of 2018, bringing a variety of knowledge, experience and enthusiasm to his new position.

Marie Candelora
Chief Operating Officer

Marie Candelora earned her Master’s of Science in Education at Alfred University in 2005 and joined the Blossom family in 2009 as an Independent Living Skills Trainer.  She spent several years working with the NHTD & TBI Waivers managing the ILST department, enrolling clients in the waivers, and chairing the Incident Review Committee.  Marie then moved on to be the Director of Marketing, where she directed and guided Blossom’s marketing initiatives, until transitioning to the role of COO.  She now oversees  the operations of Blossom. Marie has been an active member of the Greater Rochester Area Partnership for the Elderly for several years and currently serves on their Board as President.

Laura Zanino

Manager of Information Systems

Laura Zanino brings over 30 years of administrative experience to Blossom and started with the agency in 2013. She rose in the company through multiple positions to her current role as Manager of Information Systems. In this capacity, she overseas the Administrative and IT departments and is responsible for maintenance and compliance of all Electronic Medical Records for Licensed Home Care Services. She also leads the IT department in ensuring the infrastructure and security technologies are up-to-date and running smoothly. Additionally, Laura was the project manager for the successful implementation of a new agency management software program in 2021.
